    The guy who owns it posted pics with descriptions here:

    He says:
    Innovation "Mortal Kombat III Kontrol Pad". Built-in special moves buttons, swap included cartridges to unlock 250 combos (every fatality, babality, animality, friendship, pits, etc.). Two versions of controller w/ 3 cartridges each or buy carts separately. Turbo, slow-mo, character select slider.
    Apparently they're for the top-left controller, which also says "Mortal Kombat 3"

    I couldn't get you a better pic because I saw one of those thingies in a game store, just sitting on a shelf all alone. Apparently, an employee found it in the basement of the store, and nobody bought it in over a year since then.
