    If they reach 0 counters, they are sent to the graveyard after their ability resolves, if tey reach 0 because of damage, they also go to the graveyard

    Not quite right. They'll be put into your graveyard as a state-based action, which happen just after you play the ability, long before it actually resolves.

    True. Just to clarify, the ability does continue to resolve, even though the planeswalker goes to the graveyard.

    Yup. In accordance with one of Magic's golden rules - removing the source of an ability on the stack doesn't affect the ability in any way.
