When someone plays a creature, an enchantment, a sorcery/instant/interrupt (they don't exist anymore I guess), a planeswalker, etc...if you have a counterspell in your hand, you just pay the mana cost that is stated in the upper right hand corner and say "counter it"...and then if your opponent doesn't have any counters on the stack, the spell that you are countering is put into the graveyard after the stack resolves...
Almost. When you receive priority after an opponent plays a spell (anything other than a land), you play the counterspell just as you would play any other spell, targeting the spell your opponent played. When the counterspell resolves, the targeted spell is put into its owner's graveyard without taking effect.
